where are you right now? If you are in India, you just need to lodge a police report and you can apply for a duplicate passport. At least you must be having the passport number right? or just provide them the file number which you must have got when you applied for the passport in the first place. And if you are outside India, visit any Indian consulate in that country.
You forgot to mention where you're from...
Anyways, For India, File an FIR and submit it with the Passport Authorities along with your ID proof that you submitted while applying for the Passport.
